2012-2013 Immunization Requirements

ELEMENTARY SCHOOL
REQUIREMENTS FOR GRADE 3
For the 2012-2013 school year, students in Grade 3 will need to have had the following:

1. Three doses of hepatitis B vaccine (Hep B)
2. Five doses of diphtheria, tetanus & pertussis vaccine (DTaP)
3. Four doses of polio vaccine (IPV)
4. Two doses of measles, mumps & rubella vaccine (MMR)
5. One dose of chickenpox vaccine (Varicella or Varivax) OR history of having had the disease

REQUIREMENTS FOR GRADES K – 2
For the 2012-2013 school year, students in Grades K through 2 will need to have had the following:

1. Three doses of hepatitis B vaccine (Hep B)
2. Five doses of diphtheria, tetanus & pertussis vaccine (DTaP)
3. Four doses of polio vaccine (IPV)
4. Two doses of measles, mumps & rubella vaccine (MMR)
5. Two doses of chickenpox vaccine (Varicella or Varivax) OR history of having had the disease

If your student needs any or all of the above vaccines, please make an appointment as soon as possible with your family physician or with the Hancock County Health Department (477-1125).

Once your student has received any new vaccines, the school needs to have the medical documentation, no later than the start of the 2012-2013 school year. You may mail or bring it to the School Administration Office this summer; the address is 110 W. North St., Greenfield, Indiana 46140.
