ELEMENTARY SCHOOL
\nREQUIREMENTS FOR GRADE 3
\nFor the 2012-2013 school year, students in Grade 3 will need to have had the following:<\/p>\n
1. Three doses of hepatitis B vaccine (Hep B)
\n2. Five doses of diphtheria, tetanus & pertussis vaccine (DTaP)
\n3. Four doses of polio vaccine (IPV)
\n4. Two doses of measles, mumps & rubella vaccine (MMR)
\n5. One dose of chickenpox vaccine (Varicella or Varivax) OR history of having had the disease<\/p>\n
REQUIREMENTS FOR GRADES K – 2
\nFor the 2012-2013 school year, students in Grades K through 2 will need to have had the following:<\/p>\n
1. Three doses of hepatitis B vaccine (Hep B)
\n2. Five doses of diphtheria, tetanus & pertussis vaccine (DTaP)
\n3. Four doses of polio vaccine (IPV)
\n4. Two doses of measles, mumps & rubella vaccine (MMR)
\n5. Two doses of chickenpox vaccine (Varicella or Varivax) OR history of having had the disease<\/p>\n
If your student needs any or all of the above vaccines, please make an appointment as soon as possible with your family physician or with the Hancock County Health Department (477-1125).<\/p>\n
Once your student has received any new vaccines, the school needs to have the medical documentation, no later than the start of the 2012-2013 school year. You may mail or bring it to the School Administration Office this summer; the address is 110 W. North St., Greenfield, Indiana 46140.<\/p>\n
